The setting is an alternate future that diverges from our reality in the late '80s. The narrative unfolds in the Intergalactic universe where a new religion emerges and swiftly becomes dominant. Naughty Dog dedicated several years to crafting its lore, meticulously detailing the journey from its first prophet to the subsequent transformations and distortions that follow.

This religion originates and spreads on a single planet, which eventually becomes completely isolated from the rest of the galaxy. The protagonist crash-lands on this very planet, finding herself in a desolate place where she is the only human. Survival becomes the central theme of the game, with the protagonist needing to navigate this solitary environment alone. Druckmann noted that, unlike previous Naughty Dog games that often included a companion for the player to interact with, Intergalactic: The Heretic Prophet requires players to solve challenges and unravel the mysteries of the planet without assistance, emphasizing a deeply personal journey of survival and escape.
